Supportive Living & Housing Programs
Crystal Care Group operates a range of supportive living and housing programs designed to meet individuals at different stages of stability and recovery
- Enhanced Support Homes (Addictions & Mental Health – AMH): Recovery-focused environments for adults and seniors requiring higher levels of supervision and structured support
- Transition/Independent Housing for Adults: Focused on skill-building, independence, and transition into stable housing
All programs are delivered through a holistic, recovery-oriented framework, supporting residents across the eight dimensions of wellness.
Supportive Living & Group Homes
Our supportive living residences provide structured, secure environments for individuals requiring enhanced supervision and care.
Residents are supported to:
- Achieve emotional and mental stability
- Improve physical health and safety
- Develop daily living and self-care skills
- Strengthen social relationships and community belonging
- Build life skills, purpose, and hope
- Increase independence and informed decision-making
Core supports include:
- 24/7 on-site staffing and supervision
- Assistance with activities of daily living
- Behavioural support and crisis intervention
- Medication support and health monitoring
- Culturally responsive and spiritually respectful care
Individualized Support Planning
- Following intake, the Recovery Alberta Case Manager develops a support plan in partnership with CCG managementidentifying unmet needs, care priorities, and recovery goals. Plans are implemented, monitored, and adjusted over time to reflect the resident’s evolving needs and strengths.
- Clear communication protocols are established among staff, residents, families, and service partners to promote trust, transparency, and collaboration.
Wrap-Around & On-Demand Supports
Recognizing that recovery is not linear, CCG provides flexible wrap-around and on-demand supports, including:
- Life skills development and community integration
- Emotional and psychosocial support
- Crisis stabilization and short-term intensive interventions
- Support with social, cultural, and spiritual connections
- Coordination with healthcare, justice, and social service systems
Care Coordination & Case Management
Our recovery-oriented, strengths-based care coordination model ensures residents are supported as whole individuals.
We work collaboratively to:
- Align care plans with the eight dimensions of wellness
- Support residents in setting realistic, meaningful goals
- Promote autonomy, dignity, and informed choice
- Ensure continuity of care across systems
